EZ-Log Sign In

The EZ-Log application and patient data are protected by a unique, dual-level username and password combination, one for the EZ-Log application (or other functions of the handheld device), and the other for the EZ-Log patient data. Only patient ID numbers, assigned by the clinic during the enrollment process, are displayed on the EZ-Log PDA and the EZ-Log Web Application.

After a successful login, your EZ-Log PDA communicates with the EZ-Log secure database over a telephone line, using the cradle modem. If you want to access the web-based application, you can sign in through an Internet connection, using your unique preset user ID and password.

Kogenate® FS, Antihemophilic Factor (Recombinant), is a recombinant factor VIII treatment indicated for the treatment of hemophilia A. The most frequently reported adverse events were local injection site reactions, dizziness, and rash. Known intolerance or allergic reactions to constituents of the preparation is a contraindication to the use of Kogenate® FS. Known hypersensitivity to mouse or hamster protein may be a contraindication to the use of Kogenate® FS. For important safety and use information, please see the full prescribing information.
