Kogenate® FS Reliance Program
Sometimes a patient is covered for medications such as Kogenate FS under a private health insurance plan (such as a health plan offered by an employer) but a lapse in coverage occurs. In this situation, the Kogenate® FS Reliance Program offers temporary support to qualified patients.
The Kogenate® FS Reliance Program is a coupon-based program designed to ensure continuous treatment in the event of a lapse in private health insurance coverage. For every three months of insured reimbursement for Kogenate FS, the Bayer Kogenate® FS Assurance Program (BKAP) will issue one coupon. A maximum of four coupons per year may be issued.
Patients must have received treatment with Kogenate FS for one year and must have saved a minimum of four coupons to be eligible for coupon redemption. Once four coupons are saved (representing one continuous year of previous Kogenate FS treatment), the eligible patient may redeem the coupons for Kogenate FS in the event of a lapse in private health insurance.
Eligible patients should enroll in the program immediately to begin collecting coupons. Patients who enroll at the time of an insurance lapse will not be able to redeem coupons since a minimum of one year of continuous Kogenate FS treatment is required. Please encourage patients to call the Bayer Reimbursement HELPline⢠as soon as possible to begin the application and enrollment process.
Eligibility
- Only patients receiving coverage for Kogenate® FS through a private health insurance plan will be considered for the Kogenate® FS Reliance Program.
- Patients covered by Medicare, Medicaid, or other federal and state programs are not eligible.
- Due to the regulations of certain states, some privately insured patients are not eligible for the program. Please call the HELPline⢠(800.288.8374) to find out if your state has restrictions.
